Background: Low oxygen saturation (LOS) is a frequent occurrence for patients with post-tuberculosis tracheobronchial stenosis (PTTS) during bronchoscopic procedures. However, there are currently no systematic assessment tools to predict LOS risk in PTTS bronchoscopy. Objectives: This study aimed develop an effective preoperative predictive model guide clinical practice. Design: Retrospective cohort study. Methods: Data was retrospectively collected from who underwent interventions between January 2017 and December 2022. Among all included this study, 2021 were used as training the logistic regression model, 2022 utilized validation internal validation. Traditionally, the diagnosis of acute rejection (AR) relies on invasive transbronchial biopsies (TBBs) to obtain histopathological samples. We aimed evaluate diagnostic yield probe-based confocal laser endomicroscopy (pCLE) as a complementary and non-invasive tool for ACR screening, comparing its results with those obtained from TBBs. Between January 2015 April 2022, we conducted retrospective study all lung transplant recipients aged over 18 years at Toulouse University Hospital (France). All patients who underwent bronchoscopies both TBBs pCLE imaging were included. Two experienced interpreters (TV MS) reviewed images independently, blinded clinical information pathology results. From 120 procedures in 85 patients, 34 abnormal histological samples identified. Probe-based revealed significant associations between alveolar (ALC) perivascular (PVC) cellularities (p<0.0001 0.003 respectively). Alveolar cellularity demonstrated sensitivity (Se) 85.3 %, specificity (Spe) 43 positive predictive value (PPV) 37.2 % negative (NPV) 88.1 %. For PVC, Se was 70.6 Spe 80.2 PPV 58.5 NPV 87.3 Intra-interpreter correlation (TV) 88.3 number vessels (+/-1), 98.3 ALC 90 PVC. Transbronchial lung biopsy guided by radial probe endobronchial ultrasonography with a guide sheath (EBUS-GS-TBLB) is becoming significant approach for diagnosing peripheral pulmonary lesions (PPLs). We aimed to explore the clinical value of resistance pass through lesion in diagnosis PPLs when performing EBUS-GS-TBLB, and determine optimum number EBUS-GS-TBLB.We performed prospective, single-center study 126 consecutive patients who underwent EBUS-GS-TBLB solid positive-bronchus-sign where was located within from September 2019 May 2022. The classification each carried out two bronchoscopists independently, final result depended on bronchoscopist responsible procedures. primary endpoint diagnostic yield according pattern. secondary endpoints were factors affecting yield.
