Abstract Background Acinetobacter baumannii is a health threat due to its antibiotic resistance. Herein, susceptibility and association with the Toxin-antitoxin (TA) system genes in A. clinical isolates from Iran were investigated. Next, we prepared meropenem-loaded chitosan nanoparticles (MP-CS) investigated their antibacterial effects against meropenem-susceptible bacterial isolates. Methods Out of 240 specimens, 60 assessed. Antibiotic resistance conventional antibiotics was determined alongside investigating presence three TA ( mazEF , relBE higBA ). Chitosan characterized terms size, zeta potential, encapsulation efficiency, meropenem release activity. Their assessed using well diffusion method, minimum inhibitory concentration (MIC), colony-forming unit (CFU) counting. cytotoxic biocompatibility index via MTT, LDH, ROS formation assays. Results Ampicillin, ceftazidime, colistin least effective, amikacin tobramycin most effective antibiotics. isolates, 10 (16.7%), 5 (8.3%), 45 (75%) multidrug-resistant (MDR), extensively drug-resistant (XDR), pandrug-resistant (PDR), respectively. had no significant effect on MP-CS demonstrated an average size 191.5 potential 27.3 mV maximum efficiency 88.32% rate 69.57%. mediated similar effects, as compared free meropenem, significantly lower levels meropenem. remarkably prevented A549 NCI-H292 cell infection by demonstrating favorable index. Conclusion Antibiotic-loaded should be further designed increase assess safety applicability vivo settings.
