Abstract Background: Restless arms syndrome (RAS) is the most common variant of restless legs (RLS), which easy to be ignored in clinical practice due lack specific diagnostic criteria. When effective therapeutic agents induced RAS and symptoms persisted after briefly observation, clinicians will face challenge weighing efficacy against side effects. Case presentation:A 67-year-old woman was admitted a geriatric psychiatric ward with depression. Upon admission, escitalopram dose reduced from 15 mg 10 per day, duloxetine increased 60 80 day. The next night before bedtime, she developed itching creeping sensations deep inside bilateral shoulders arms, urge move, worsening at rest, alleviation hammering. when discontinued. A history RLS confirmed. Treatment 40 0.125 pramipexole significantly improved depression, paresthesia disappeared, no recurrence occurring 6 months discharge. Discussion conclusions: This case suggests that psychiatrists should pay attention variants increasing doses duloxetine. Long-term improvement can achieved through dosage reduction combined dopaminergic drugs instead immediate discontinuation.

Depression can trigger an inflammatory response that affects the immune system, leading to development of other diseases related inflammation. Xiao-Yao-San (XYS) is a commonly used formula in clinical practice for treating depression. However, it remains unclear whether XYS has modulating effect on associated with The objective this study was examine role and mechanism regulating anti-inflammatory A chronic unpredictable mild stress (CUMS) mouse model established evaluate antidepressant effects XYS. Metabolomic assays network pharmacology were utilized analyze pathways targets its effects. In addition, molecular docking, immunohistochemistry, Real-Time Quantitative Polymerase Chain Reaction (RT-qPCR), Western Blot performed verify expression relevant core targets. results showed significantly improved depressive behavior attenuated CUMS mice. analysis revealed reversible modulation 21 differential metabolites by depression-related Through combination liquid chromatography pharmacology, we identified seven active ingredients key genes. Furthermore, integrating predictions from findings metabolomic analysis, Vascular Endothelial Growth Factor (VEGFA) Peroxisome Proliferator-Activated Receptor-γ (PPARG) as Molecular docking experiments confirmed these results. present employed metabolomics analyses provide evidence ability alleviate depression through multiple metabolic
