An important mechanism for the development of intervertebral disc degeneration (IDD) is an imbalance between anti-inflammatory and pro-inflammatory cytokines. Therapeutic non-therapeutic approaches cytokine correction in IDD either do not give expected result, or a short period time. This explains relevance high-tech medical care, which part specialized care includes use new resource-intensive methods treatment with proven effectiveness. The aim review to update knowledge about based on IDD. It demonstrates promise management patients resistant previously used therapies, including: cell therapy (stem implantation, implantation autologous cultured cells, tissue engineering); genetic technologies (gene modifications, microRNA, molecular inducers IDD); influencing inflammatory cascade discs mediated by abnormal activation inflammasomes; senolytics; exosomal therapy; other factors (hypoxia-induced factors; lysyl oxidase; corticostatin; etc.).

Intervertebral disc degeneration (IVDD) constitutes a crucial pathological foundation for spinal degenerative diseases (SDD) and stands as primary contributor to both low back pain (LBP) disability. The progression of IVDD is linked structural functional alterations in tissues, where an imbalance the inflammatory microenvironment can induce extracellular matrix (ECM) degradation, senescence, apoptosis. This key pathomechanism disease's development, gaining considerable attention recent years. study aims conduct bibliometric analysis publications pertaining mechanisms quantitatively assess current research hotspots directions.

Intervertebral disk degeneration (IVDD) is a common spine disease, and inflammation considered to be one of its main pathogenesis. Apigetrin (API) natural bioactive flavonoid isolated from various herbal medicines shows attractive anti-inflammatory antioxidative properties; whereas, there no exploration the therapeutic potential API on IVDD. Here, we aim explore role IVDD in vivo vitro.

Purpose: To investigate the molecular mechanism underlying inhibitory effect of sinomenine (SN) on interleukin-1β (IL-1β)-induced apoptosis in nucleus pulposus cells (NPCs), and to evaluate potential role SN preventing intervertebral disk degeneration (IDD). Methods: The Rat NPCs were cultured vitro identified using Hematoxylin-Eosin (HE) staining, toluidine blue immunofluorescence analysis. pretreated with or without SN, then induced IL-1β assess cell viability, ROS levels, apoptotic rates, wound healing ability. Relevant protein expression was detected Elisa, qPCR Western Blot techniques. either alone combination Nrf2-IN-1 SC, before being undergo by IL-1β. Apoptosis Hoechst while techniques assessed expression. caudal discs IDD, injection, co-injected levels IDD evaluated HE staining modified saffron-O-fix green cartilage staining. qPCR, Results: significantly reduced NPC activity, accumulation apoptosis, decreased rate, promoted secretion inflammatory factors, inhibited extracellular matrix synthesis. However, pretreatment effectively reversed these effects. Inhibition Keap1/Nrf2 signaling pathway activation NF-κB attenuated cytoprotective effects increased apoptosis. Acupuncture combined injection markedly disc rat spine, upregulated factors secretion, downregulated intervention notably enhanced antioxidant enzyme outcomes. Conclusion: can prevent IL-1β-induced ameliorate activating inhibiting pathway. Keywords: sinomenine, degeneration, cells, Nrf2,
