Objective. This paper presents a novel dual-branch framework for estimating blood pressure (BP) using photoplethysmography (PPG) signals. The method combines deep learning with clinical prior knowledge and models different time periods (morning, afternoon, evening) to achieve precise, cuffless BP estimation.Approach.Preprocessed single-channel PPG signals are input into two feature extraction branches. first branch converts dimensions 2D uses pre-trained Mobile Vision Transformer-v2 (MobileViTv2) Visual Geometry Group19 (Vgg19) backbones extract features based on the mechanisms of systolic (SBP) diastolic (DBP) formation. second calculates multi-dimensional parameters relationship between waveforms factors affecting BP. We fuse from both branches consider diurnal variations, AutoML strategy construct specific SBP DBP estimation periods. algorithm was developed human resting state dataset (HRSD) validated MIMIC-IV generalization performance.Main results. The mean absolute error (MAE) is 6.42 mmHg 4.96 in morning, 4.84 3.73 2.65 2.56 evening. Asymptomatic peripheral artery disease (PAD) poses a silent risk, potentially leading to severe conditions if undetected. Integrating new screening tools into routine general practitioner (GP) visits could enable early detection. This study investigates the feasibility of photoplethysmography (PPG) monitoring for assessing vascular health across different blood pressure (BP) conditions. Custom femoral phantoms representing healthy (0.82 MPa), intermediate (1.48 and atherosclerotic (2.06 MPa) vessels were tested under hypertensive, normotensive, hypotensive evaluate PPG’s ability distinguish between states. Extracted features from PPG signal, including amplitude, area curve (AUC), median upslope–downslope ratio, end datum difference, analysed. Kruskal–Wallis tests revealed significant differences unhealthy BP states, supporting as tool. The fiducial points second derivative signal (SDPPG) ba ratio was most pronounced hypertensive (ranging –2.13 –2.06), suggesting change in wall distensibility. Under normotensive conditions, difference ratios smaller (0.01), no meaningful observed reduced sensitivity this metric at lower perfusion pressures. Intermediate states challenging detect, particularly hypotension, need further research. Nonetheless, highlights promise identifying stiffness.
