Importance Associations between child maltreatment (CM) and health have been studied broadly, but most studies focus on multiplicity (number of experienced subtypes CM). Studies assessing multiple CM characteristics are scarce, partly due to methodological challenges, were mostly conducted in patient samples. Objective To determine the importance association with physical multimorbidity adulthood for women men a German representative sample. Design, Setting, Participants This survey study used data from sample population July October 2021. Households randomly selected within regional areas by demographic consultation company, Kish-Grid method was applied ensure random participation. Reasons nonparticipation refusal household or target person participate failure contact after 4 visits. Analyses took place June 2023 2024. Exposure Characteristics CM, including multiplicity, age at time (timing), number years (duration), frequency, subjective severity (measured ISPCAN Child Abuse Screening Tools Retrospective version questionnaire). Main Outcomes Measures The main outcome multimorbidity, defined as sum score lifetime leading morbidity mortality causes Western countries (obesity, diabetes, cancer, hypertension, myocardial infarction, chronic obstructive pulmonary disease, incident stroke). Conditioned forest regression analyses (a machine learning technique) examine what importantly associated while considering all other variables model. Results Of 5908 individuals invited, included 2514 participants (response rate, 42.6%), 1297 (51.6%) (mean [SD] age, 50.6 [17.9] years) 1217 (48.4%) 49.5 [18.2] years). Duration important factor both (importance = 0.595; 95% CI, 0.599-0.601) 1.389; 1.386-1.394). timing more outcomes than men. For women, experiencing significantly adulthood. men, 11 Conclusions Relevance In this study, conditioned provide insights duration addition frequently multiplicity. These findings suggest that assessments should be considered diagnostics conditions may also inform strategies mitigate risk.

BackgroundChildhood maltreatment is associated with poor mental and physical health. However, the mechanisms of gene–environment correlations potential causal effects childhood on health are unknown. Using genetics, we aimed to delineate sources correlation for relationship between health.MethodsWe did a genome-wide association study meta-analysis using data from UK Biobank (n=143 473), Psychiatric Genomics Consortium (n=26 290), Avon Longitudinal Study Parents Children (n=8346), Adolescent Brain Cognitive Development (n=5400), Generation R (n=1905). We included individuals who had phenotypic genetic available. investigated single nucleotide polymorphism heritability among different subtypes, operationalisations, reports maltreatment. Family-based population-based polygenic score analyses were done elucidate mechanisms. used Mendelian randomisation identify shared genetics test relationships conditions.FindingsOur studies (N=185 414) identified 14 independent loci (13 novel). high overlap (genetic 0·24–1·00) reporting methods. Robust suggested role in depression (unidirectional), as well both schizophrenia ADHD (bidirectional), conditions (coronary artery disease, type 2 diabetes) or inflammation (C-reactive protein concentration).InterpretationChildhood has heritable component, substantial retrospective prospective point correlation, equivocal supports (primarily bidirectional) health, conditions. Background: Somatic symptoms, such as chronic pain, fatigue, and gastrointestinal disturbances, are commonly reported in individuals with a history of childhood maltreatment (CM), which includes various forms abuse neglect experienced before age 18. Although CM is strongly associated somatic the specific relationships between subtypes these well mechanisms connecting them, remain insufficiently understood. This review examines complex interaction often coexist mental disorders significantly impact quality life healthcare systems. Summary: frequently mix "explained" "unexplained" conditions, personal distress pose diagnostic challenges. has been linked to symptoms through neurobiological mechanisms, HPA axis dysregulation allostatic load, while theoretical models emphasize roles hyperawareness, cultural factors, vulnerability symptom development. However, existing research fails account for subtypes, full range situational leading inconsistencies findings. Key Messages: Bridging gaps literature requires adopting World Health Organization’s subtype definitions ICD-11 codes (MA00-MH2Y) encompass broader spectrum symptoms. Employing rigorous methodologies, systematic reviews meta-analyses, essential advancing understanding. These approaches can enhance accuracy, support tailored interventions, promote biopsychosocial framework research, ultimately improving patient outcomes alleviating societal burdens.

Alterations in hypothalamic-pituitary-adrenal (HPA) axis and its effector hormone cortisol have been proposed as one possible mechanism linking child maltreatment experiences to health disparities. In this series of meta-analyses, we aimed quantify the existing evidence on effect various measures HPA activity. The systematic literature search yielded 1,858 records, which 87 studies (k = 132) were included. Using random-effects models, found for blunted stress reactivity individuals exposed maltreatment. contrast, no overall differences any other activity (including daily activity, assessed context pharmacological challenges cumulative secretion). impact several moderators (e.g., sex, psychopathology, study quality), role methodological shortcomings studies, well potential directions future research are discussed.

Abstract Background Parents and caregivers belonged to those with the highest burdens during COVID-pandemic. Considering close link between parental stress child maltreatment, identifying families high is of utmost importance prevent violence against children. Within this study, we thus aimed investigate interplay stress, changes in physical children second year COVID-pandemic on an exploratory level. Methods We conducted a cross-sectional, observational study Germany from July October 2021. By using different sampling steps, representative probability sample German population was generated. A subsample these participants under age 18 included for analysis within (N = 453, 60.3% females, M 40.08; SD 8.53). Results Higher levels were associated more children, higher own experiences mental health symptoms. An increase pandemic female sex, use experience maltreatment. who have ever used their been characterized by levels, stronger pandemic, symptoms sociodemographic characteristics. having pre-existing psychiatric disorders, maltreatment predicted increased pandemic. Conclusions Our results underscore risk so times overall due underline need low threshold support at crises.

ABSTRACT Objective Childhood trauma is a key public health risk factor for developing physical illness. This study examined how childhood abuse and neglect predict adult somatic symptoms assessed retrospectively in real time. Methods Participants the HowNutsAreTheDutch project completed Trauma Questionnaire—Short Form and, to assess symptoms, cross-sectional Patient Health Questionnaire ( n = 406) or longitudinal ecological momentary assessment protocol that included responding an item on discomfort 3 times day 30 days 290). Results As hypothesized, was positively associated with using d 0.30) 0.31), also after adjustment age, sex, educational level, relationship status. Longitudinally, we explored whether interpersonal context moderated outcomes, observed social company dampened among people history of more 0.04). Conclusions trauma, especially emotional sexual abuse, predicts specific symptoms. Future work may examine symptom expression influenced by lives traumatized individuals.
