Endometriosis is a common disorder associated with pain, gastrointestinal and urinary symptoms, infertility, fatigue. It defined by the presence of endometrial-like lesions found predominantly in pelvis. Mechanisms that contribute to disease aetiology include changes hormonal, inflammatory, pain pathways. In this article, we focus on recent developments imaging technologies, our improved understanding mechanisms contributing drug therapies are clinical trials, insights from studies gut offer potential support self-management strategies. We postulate improvements quality life patients will be accelerated reframing endometriosis as multi-system learning treatments targeting symptoms shared between endometriosis, neuroinflammatory, disorders.

Abstract Background In recent years, fibroblast activating protein (FAP), a biomarker overexpressed by cancer-associated fibroblasts, has emerged as one of the most promising biomarkers in oncology. Similarly, FAP overexpression been detected various fibroblast-mediated inflammatory conditions such liver cirrhosis and idiopathic pulmonary fibrosis. Along this trajectory, FAP-targeted positron emission tomography (PET), utilizing inhibitors (FAPi) labeled with emitters, gained traction powerful imaging approach both cancer inflammation. However, PET represents high-cost technology, its widespread adoption is still limited compared to availability gamma cameras. To address issue, several efforts have made explore potential [ 99m Tc]Tc-FAPi tracers molecular probes for cameras single photon computed (SPECT). Main body Several approaches investigated labeling FAPi-based compounds Tc. Specifically, mono-oxo, tricarbonyl, isonitrile, HYNIC strategies applied produce tracers, which tested vitro animal models. Overall, these demonstrated high efficiency strong binding. The resulting shown specificity FAP-positive cells xenografts model studies, respectively. majority exhibited variable levels lipophilicity, leading preferential excretion through hepatobiliary route undesirable binding lipoproteins. Consequently, synthesize more hydrophilic improve pharmacokinetic properties achieve favorable biodistribution, particularly abdominal region. SPECT yielded results patients gastrointestinal tumors, demonstrating comparable or superior diagnostic performance other modalities. encouraging outcomes observed subjects gliomas, lung cancer, breast cervical cancer. Beyond oncological applications, Tc]Tc-FAPi-based successfully employed myocardial Conclusions This overview focuses on radiochemical obtaining highlighting main challenges encountered possible solutions when applying each distinct approach. Various papers have introduced the use of positron emission tomography (PET) with [68Ga]Ga-radiolabeled fibroblast-activation protein inhibitor (FAPi) radiopharmaceuticals in different subtypes gastric cancer (GC). Our aim was to assess diagnostic performance this novel molecular imaging technique GC a systematic review and meta-analysis. A straightforward literature search concerning FAP-targeted PET performed. Original articles evaluating examination both newly diagnosed patients disease relapse were included. The included nine original studies, eight them also eligible for quantitative synthesis provided pooled detection rates 95% 97% assessment primary tumor distant metastases, respectively, sensitivity specificity 74% 89%, regional lymph node metastases. Significant statistical heterogeneity among studies found only analysis rate (I2 = 64%). Conclusions: Beyond limitations meta-analysis (i.e., all conducted Asia, using [18F]FDG PET/CT as comparator index test), data demonstrate promising GC. Nevertheless, more prospective multicentric are needed confirm excellent performances cluster patients.
