Abstract Since the discovery of immunoglobulin E (IgE) as a mediator allergic diseases in 1967, our knowledge about immunological mechanisms IgE‐mediated allergies has remarkably increased. In addition to understanding immune response and clinical symptoms, allergy diagnosis management depend strongly on precise identification elicitors reaction. past four decades, innovations bioscience technology have facilitated production well‐defined, highly pure molecules for component‐resolved (CRD), allowing personalized disease individual patients. The first edition “EAACI Molecular Allergology User's Guide” (MAUG) 2016 rapidly became key reference clinicians, scientists, interested readers with background allergology, immunology, biology, medicine. Nevertheless, field molecular allergology is moving fast, after 6 years, new EAACI Taskforce was established provide an updated document. Guide 2.0 summarizes state‐of‐the‐art information allergen molecules, their relevance, application diagnostic algorithms practice. It designed both, clinicians guiding health care professionals through overwhelming list different available testing. Further, it provides relevance allergenic gives overview basic test formats, tests measure exposure.

Background: Lipid transfer proteins (LTP) are the most common food allergens in Mediterranean region. Objective: The study aimed to investigate co-sensitization patterns and cluster relationships between LTP allergen molecules across a broad range of allergen-specific sensitization patterns, clinical outcomes eastern children. Methods: Among 496 children evaluated for multiple sensitizations with multiplex testing, 105 (21%) 16 different were analyzed. Clinical reactivity was examined based on clear-cut history immunoglobulin E mediated symptoms (oral allergy syndrome [OAS], systemic reactions, anaphylaxis). Results: All included sensitive LTPs, but 56% pollen LTPs. number OAS 12 59, respectively, no cofactors reported. Pru p 3 (74%) Cor 8 (66%). Significant correlations observed heatmap other than Par j 2 Tri 14. Overall, associated increased age molecule positivity. Conclusion: In region, 21% and/or found have sensitization; however, almost half reported reactivity. hierarchical pathway highlights that distinct can act as primary sensitizers. is linked increasing numbers positivity age.

ImmunoCAP ISAC E112i (ISAC) and Allergy Explorer 2 (ALEX2) detect specific immunoglobulin E (IgE) reactivity. Both multiplex assays contain molecular allergens ALEX2 additionally includes allergen extracts inhibitors that block the binding of IgE to cross-reacting carbohydrate determinants (CCDs). This study aimed compare performance by determining reactivity against and/or components using qualitative, semiquantitative, quantitative analyses all comparable in sera from 216 participants recruited South Tyrol/Italy. For extract sensitization ALEX2, analysis revealed negative corresponding 18.4% at least one positive component 81.6% cases. ISAC, results were 23.5% 76.5% cases, respectively. The CCD inhibitor eliminated CCD-positive signals detected 88.5% Based on values 0.3–14.9 ISU or kUA/L, there was good agreement between although showed higher than ISAC. addition allergen-extract tests resulted detection more sensitizations with alone. In range <15 may be effective detecting sensitizations.
