Major depressive disorder (MDD) is often marked by impaired motivation and reward processing, known as anhedonia. Many patients do not respond to first-line treatments, improvements in can be slow, creating an urgent need for rapid interventions. Recently, we demonstrated that transcutaneous auricular vagus nerve stimulation (taVNS) acutely boosts effort invigoration healthy participants, but its effects on depression remain unclear.

Introduction: Appraisal of prodromal symptoms unipolar depression may complement the traditional cross-sectional approach and provide a longitudinal perspective, according to staging model illness. Objective: To an updated systematic review clinical studies concerned with depression, PRISMA guidelines. Methods: Keyword searches were conducted in PubMed, Scopus, Web Science. Longitudinal on signs adult patients primarily diagnosed selected. Findings examined separately study design (i.e., retrospective or prospective). Results: Twenty-five met criteria for inclusion this review. indicate that distinct symptomatology – commonly characterized by anxiety, tension, irritability, somatic complaints exists before onset depression. The duration phase was highly variable across studies, ranging from less than month several years. Prodromal profile consistent within individuals depressive episodes. There close relationship between residual same episode. Conclusions: present addresses important, yet relatively neglected, issue deserves further investigation be immediate practical value. findings challenging insights into pathogenesis course which result more timely effective treatment recurrences. definition would benefit joint use symptom identification, biomarkers, neuroimaging.

Aims Thyroid dysfunction and metabolic disturbances are common in major depressive disorder (MDD) patients. We aimed to assess the relationship between thyroid dysfunction, disturbances, clinical symptoms Chinese first-episode, drug-naïve (FEDN) MDD patients using undirected Bayesian network methods. Methods 1718 FEDN were recruited. Serum levels of free triiodothyronine (FT3), thyroxine (FT4), stimulating hormone (TSH), anti-thyroglobulin (TgAb), peroxidases antibody (TPOAb), total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL-C), low-density (LDL-C), glucose assessed. Blood pressure body mass index measured. Hamilton Rating Scale for Depression (HAMD), Anxiety, positive subscale Positive And Negative Syndrome Scales used detect symptoms. An with EBICglasso default a directed acyclic graph (DAG) approach was conducted. Results The prevalence rates symptoms, as follows: anxiety (n=894, 52%), psychotic (171, 10%), subclinical hypothyroidism (SCH, n=1041, 61%), abnormal TgAb (n=297, 17%), TPOAb (n=438, 25%), hyperthyroidism (n=5, 0.3%), (n=3, 0.2%), hyperglycemia (n=241, 14%), hypertriglyceridemia (n=668, 39%), low HDL-C (n=429, hypercholesterolemia (421, TC (357, 21%), LDL-C (185, 11%). overweight or obesity (n=1026, 60%), hypertension (n=92, 5.4%). Both networks demonstrated serum TSH severity depression played an important role pathophysiology MDD. Conclusions may have early stage. Improving safe and effective access to ketamine therapy is of high priority given the growing burden mental illness. Telehealth-supported administration sublingual being explored toward this goal. In longitudinal study, moderately-to-severely depressed patients received four doses at home over weeks within a supportive digital health context. Treatment was structured resemble methods therapeutic psychedelic trials. Patients receiving second course treatment were also examined. Symptoms assessed using Patient Health Questionnaire (PHQ-9) for depression. We conducted preregistered machine learning symptom network analyses investigate outcomes (osf.io/v2rpx). A sample 11,441 analyzed, demonstrating modal antidepressant response from both non-severe (n = 6384, 55.8 %) severe 2070, 18.1 baseline depression levels. Adverse events detected in 3.0–4.8 % participants predominantly neurologic or psychiatric nature. helped extend improvements who responded favorably initial treatment. Improvement most strongly predicted by lower scores age baseline. Depressed mood Anhedonia sustained despite ongoing This study limited absence comparison control groups lack fixed-dose procedure administration. At-home, telehealth-supported largely safe, well-tolerated, associated with improvement Strategies combining psychedelic-oriented therapies rigorous telehealth models, as here, may uniquely address barriers
