Inflammation is an integral part of autoimmune diseases, which are caused by dysregulation the immune system. This involves imbalance between pro-inflammatory versus anti-inflammatory mediators. These mediators include various cytokines and chemokines; defined subsets T helper/T regulatory cells, M1/M2 macrophages, activating/tolerogenic dendritic antibody-producing/regulatory B cells. Despite availability many anti-inflammatory/immunomodulatory drugs, severe adverse reactions associated with their long-term use often high costs impediments in effectively controlling disease process. Accordingly, suitable alternatives being sought for these conventional drugs. Natural products offer promising adjuncts/alternatives this regard. The specific compounds isolated from dietary/medicinal plant extracts have permitted rigorous studies on disease-modulating activities mechanisms involved therein. Here, we describe basic characteristics, action, preventive/therapeutic applications 5 well-characterized natural product (Resveratrol, Curcumin, Boswellic acids, Epigallocatechin-3-gallate, Triptolide). been tested extensively animal models autoimmunity as well limited clinical trials patients having corresponding diseases. Macrophage polarization determines the production of cytokines that fuel initiation and evolution rheumatoid arthritis (RA). Thus, modulation macrophage might represent a potential therapeutic strategy for RA. However, coordinated macrophages in synovium synovial fluid has not been achieved thus far. Herein, we develop biomimetic ApoA-I mimetic peptide-modified neutrophil membrane-wrapped F127 polymer (R4F-NM@F127) targeted drug delivery during RA treatment. Due to high expression adhesion molecules chemokine receptors on neutrophils, membrane coating can endow nanocarrier with synovitis-targeting ability, subsequent recruitment under chemotactic effects IL-8. Moreover, R4F peptide modification further endows ability target SR-B1 receptor, which is highly expressed fluid. Long-term vivo imaging shows R4F-NM@F127 preferentially accumulates inflamed joints engulfed by macrophages. After loading anti-inflammatory celastrol (Cel), R4F-NM@F127-Cel significant reduction hepatotoxicity, effectively inhibits inflammation alleviates joint damage reprogramming polarization. our results highlight as promising option treatment

Objective To evaluate safety and efficacy of dietary polyphenols in the treatment rheumatoid arthritis (RA). Methods CNKI, Pubmed, Cochrane library, Embase were searched to collect randomized controlled trials (RCTs) RA. The databases from time their establishment November 8nd, 2022. After 2 reviewers independently screened literature, extracted data, assessed risk bias included studies, Meta-analysis was performed using RevMan5.4 software. Results A total 49 records (47 RCTs) finally included, involving 3852 participants 15 types (Cinnamon extract, Cranberry Crocus sativus L. Curcumin, Garlic Ginger Hesperidin, Olive oil, Pomegranate Puerarin, Quercetin, Resveratrol, Sesamin, Tea polyphenols, Total glucosides paeony). Cinnamon Sesamin only involve 1 RCT. RCTs. paeony Curcumin more than 3 These RCTs showed that these could improve disease activity score for 28 joints (DAS28), inflammation levels or oxidative stress addition did not increase adverse events. Conclusion Dietary may DAS28, reduce C-reactive protein (CRP) erythrocyte sedimentation rate (ESR), stress, etc. However, are needed verify modify polyphenols. Abstract Rheumatoid arthritis (RA) is a chronic autoimmune disease marked by synovitis and cartilage destruction. The active compound, icariin (ICA), derived from the herb Epimedium, exhibits potent anti-inflammatory properties. However, its clinical utility limited water insolubility, poor permeability, low bioavailability. To address these challenges, we developed multifunctional drug delivery system—adipose-derived stem cells-exosomes (ADSCs-EXO)-ICA to target macrophages in synovial tissue modulate macrophage polarization M1 M2. High-performance liquid chromatography analysis confirmed 92.4 ± 0.008% loading efficiency for ADSCs-EXO-ICA. In vitro studies utilizing cellular immunofluorescence (IF) flow cytometry demonstrated significant inhibition of proliferation Enzyme-linked immunosorbent assay, transcriptomics, real-time quantitative PCR indicated that ADSCs-EXO-ICA promotes an M1-to-M2 phenotypic transition reducing glycolysis through ERK/HIF-1α/GLUT1 pathway. vivo, effectively accumulated joints. Pharmacodynamic assessments revealed decreased cytokine levels mitigated symptoms collagen-induced (CIA) rats. Histological micro computed tomography markedly ameliorated preserved cartilage. Further vivo suppresses promoting switch suppressing glycolysis. Western blotting supported therapeutic efficacy RA, confirming role modulating function energy metabolism regulation.
