A systemic analysis was performed to evaluate the prognostic utility of Glasgow score (GPS) and modified (m)GPS in cancer patients treated with immune checkpoint inhibitors (ICI). Abstract Background Gastric cancer remains a major health concern worldwide, with locally advanced gastric (LAGC) posing significant challenges due to frequent lymph node metastasis and poor prognosis. While curative gastrectomy D2 dissection is the standard treatment, sarcopenia perioperative skeletal muscle loss (SML) have emerged as critical prognostic factors. Methods We retrospectively analyzed 198 LAGC patients who underwent gastrectomy. Preoperative postoperative were assessed via computed tomography (CT)-derived index (SMI) at L3 level. SML was defined based on sex-specific thresholds of SMI changes (%/30 days). Prognostic significance for overall survival (OS) disease-free (DFS) evaluated using Kaplan-Meier Cox regression analyses. Results The prevalence increased from 23.7% preoperatively 33.3% postoperatively.
