Therefore, it of utmost importance identify individuals at risk as early possible avoid long-term T2DM In this study, we developed an interpretable machine learning model leveraging baseline levels biomarkers oxidative stress (OS), inflammation, mitochondrial dysfunction (MD) for identifying developing T2DM. particular, Isolation Forest (iForest) was applied anomaly detection algorithm address class imbalance. iForest trained on the control group data detect cases high development outliers. Two models were evaluated through ten-fold cross-validation, first traditional (BMI, blood glucose (BGL) triglycerides) alone second including additional aforementioned biomarkers. The outperformed across all evaluation metrics, particularly F1 score recall, which increased from 0.61 ± 0.05 0.81 0.57 0.06 0.08, respectively. feature scores identified novel combination biomarkers, interleukin-10 (IL-10), 8-isoprostane, humanin (HN), oxidized glutathione (GSSG), revealed be more influential than outcome prediction.
