Accurate diagnosis and subsequent therapeutic options in pulmonary diseases mainly rely on imaging methods histological assessment. However, examinations are hampered by the limited spatial resolution of images most procedures that related to assessment invasive with associated complications. As a result, high-resolution technology – confocal laser endomicroscopy (CLE), which is at forefront enables real-time microscopic visualisation morphologies architectures tissues or cells has been developed resolve clinical dilemma pertaining current techniques. The evidence shown CLE potential facilitate advanced diagnostic capabilities, monitor aid tailored treatment regime for patients diseases, as well expand horizon unravelling mechanism targets diseases. In future, if can be combined artificial intelligence, early, rapid accurate will achieved through identifying automatically. promising this technique may be, further investigations required before it enter routine practice.

Fifteen dihydroartemisinin-isatin hybrids ( 5a-e and 6a-j) linked with three-carbon were designed, synthesized. The antiproliferative activity against lung cancer cell lines including drug-sensitive A549, doxorubicin-resistant A549 (A549/DOX) cisplatin-resistant (A549/DDP) was tested. cytotocivity towards normal epithelial BEAS-2B line also investigated. From the structure-activity relationship (SAR), it found that hydrogen bond donors (especially hydroxime thiosemicarbazide) at C-3 position electron-withdrawing groups (fluoro chloro) C-5 of isatin moiety beneficial for activity. A significant part them (half maximal inhibitory concentration/IC 50 : 5.72–55.52 μ M) demonstrated considerable activity, superior to dihydroartemisinin (IC 69.42–88.03 artemisinin >100 M). In particular, two 6a, e 5.72–9.84 not inferior doxorubicin 4.06 cisplatin 9.38 cells more potent than 54.32 15.10 19.74 66.89 multidrug-resistant A549/DOX A549/DDP lines. addition, showed no toxicity cells, proving their excellent selectivity profile. Furthermore, hybrid 6a possessed good stability in mouse human microsomes, as well pharmacokinetic properties. Accordingly, could serve a promising anti-lung chemotherapeutic candidate further preclinical evaluations.

An increasing number of peripheral pulmonary lesions (PPLs) requiring tissue verification to establish a definite diagnosis for further individualized management are detected due the growing adoption lung cancer screening by chest computed tomography (CT), especially low-dose CT. However, morphological PPLs remains challenging. Transbronchial cryobiopsy (TBLC) that can retrieve larger specimens with more preserved cellular architecture and fewer crush artifacts in comparison conventional transbronchial forceps biopsy (TBFB), as an emerging technology diagnosing PPLs, has been demonstrated have potential resolve clinical dilemma pertaining currently available sampling devices (e.g., forceps, needle brush) become diagnostic cornerstone PPLs. Nineteen propylene tethered dihydroartemisinin-isatin hybrids 5a-g and 6a-l were designed, synthesized, screened for their in vitro antiproliferative activity against three lung cancer cell lines, inclusive of drug-sensitive (A549), doxorubicin-resistant A549 (A549/DOX) cisplatin-resistant (A549/DDP) lines. The cytotoxicity the synthesized towards normal epithelial line (BEAS-2B) was also assessed to evaluate selectivity. structure-activity relationship (SAR) elucidated that (1) alkyloxylimino fragment at C-3 position isatin moiety more favorable than carbonyl benzoxylimino, relative contricution order methoxylimino > ethoxylimino benzoxylimino; halogen atom C-5 or C-6 could enhance activity. As the leading cause of cancer-related deaths worldwide, early detection and diagnosis are crucial to reduce mortality lung cancer. To date, peripheral pulmonary lesions (PPLs) remains a major unmet clinical need. The urgency diagnosing PPLs has driven series development advanced bronchoscopy-guided techniques in past decades, such as radial probe-endobronchial ultrasonography (RP-EBUS), virtual bronchoscopy navigation (VBN), electromagnetic (ENB), bronchoscopic transparenchymal nodule access (BTPNA), robotic-assisted bronchoscopy. However, these also have their own limitations. In this review, we would like introduce diagnostic for PPLs, with special focus on biopsy approaches guided by discussing advantages, limitations, future prospects.
