Hymenoptera venom allergy (HVA) is a potentially life-threatening condition, making accurate diagnosis crucial for identifying significant IgE sensitizations and enabling effective immunotherapy. In this review, we provide detailed overview of biomarkers the molecular IgE-mediated hypersensitivity to insect venoms in clinical practice, present, structured manner, their importance differentiating genuine versus cross-sensitizations using different diagnostic procedures. Updated algorithms are provided, along with advantages limitations approaches. Geographical variations rare species may pose further challenges diagnosing treating HVA, adding complexity HVA management. This review informs readers about performing tailored diagnostics based on allergen subsequent treatment strategies.

Background . The allergic diseases are growing everywhere. exact diagnosis of allergens becomes more and critical because it significantly affects the further treatment patient, including question about using allergen-specific immunotherapy. There several laboratory instrumental tests among which skin most available. Despite its low cost, this method is characterized by a large percentage false-positive and, importantly, false-negative results. In addition, that can be detected limited. Aim. To conduct comparative analysis effectiveness diagnostic accuracy allergen chips from different manufacturers to evaluate their applicability in molecular diseases. Results ImmunoCAP ISAC Allergy Explorer (ALEX) gradually occupying niche Both methods have benefits disadvantages. high cost difficulties supplying foreign materials sharply limit widespread use allergy profiles Russia. On other hand, personalized medicine requires doctors patient-centered care, completed existing tools. panels for various regions Russia solve problem. Identifying common clinically significant molecules each region will enable accurate allergies patient treatment. This improve medical care quality increase therapy effectiveness. Conclusion. Developing implementing new creating optimal important steps towards improving These measures help clinicians accurately identify relevant develop individual plans ultimately leading better public health lower economic costs treating allergies.

IgE-mediated wheat allergy can take on various forms, including childhood food to wheat, wheat-dependent exercise-induced anaphylaxis in young adults, baker’s respiratory allergy/asthma workers exposed flour inhalation, and contact urticaria that is caused by hydrolyzed proteins some cosmetics, sometimes associated with a allergy. Singleplex multiplex immunoassays detect specific IgE antibodies allergenic molecular biomarkers such as omega-5 gliadin Tri 19, lipid transfer protein 14, alpha-amylase inhibitors. The fluorescence enzyme immunoassay capsulated cellulose polymer solid-phase coupled allergens commonly used singleplex assay. Multiplex methods include the ELISA-based macroarray using nano-bead technology microarray polymer-coated slides. Another promising diagnostic tool basophil activation test performed other types. Detailed comprehension of structural immunological features numerous significant clinical settings imperative for advancing allergies.

Introduction: Food-dependent, exercise-induced anaphylaxis (FDEIA) is a rare type of with growing incidence. Although the precise mechanism by which patient reacts only in combination culprit food and cofactors are not currently understood, many advances diagnosis management have been made since their first description. Areas covered: A literature search PubMed was performed to review FDEIA. Clinicians should high level suspicion for identification foods involved. Component-resolved more accurate provocation tests revolutionized accuracy. Management easy involves educating evict exposure that elicit anaphylaxis, how act treat if reaction occurs. Expert opinion: FDEIA misdiagnosed authors believe there patients labelled as idiopathic unnecessary evictions poor quality life because fear an imminent reaction. Due recent diagnostic tools use monoclonal antibodies prophylaxis persistent cases, can better prognosis improving families.

The measurement of specific IgE to allergenic extracts and molecules in patients with allergic rhinitis (AR) is crucial for a precise diagnosis further immunotherapy. Companies providing vitro diagnostic methods allergology continuously strive the optimization modernization such methods. A new generation automated allergy tests based on chemiluminescence detection paramagnetic microparticles now available, possible advantages sample volume, cost-effectiveness avoidance sample-related interference. To test whether sIgE antibody levels obtained singleplex chemiluminescent method have good agreement corresponding results "gold standard" test.We tested sera from 368 AR patients. Specific (kU/L) comprehensive panel 15 allergen 6 were ImmunoCAP® (Thermo Fisher Scientific Inc, Phadia AB, Uppsala, Sweden) NOVEOS™ (HYCOR® Biomedical, Garden Grove, CA, USA). We evaluated qualitative quantitative performance NOVEOS system matching outcome ImmunoCAP each examined allergens. In relation ImmunoCAP, overall sensitivity specificity 90.8% (95% CI = 88.6-92.7) 96.2% 93.9-97.8), respectively. These values higher when only considered (sensitivity 98.7% [95% 96.4%-99.7%]; 94.2% 88.4%-97.6%]) lower 87.6% 84.7%-90.2%]; 97% 94.4%-98.6%]). Spearman's correlation between data set both ≥ 0.1 kU/L cut-off was 0.84 (p < .001).The presented comparisons testing serum antibodies against range 21 allergens. This novel immunoassay using 4 µl per appears be robust reliable can, therefore, used as an aid diagnosis.
